I am contracted with the following insurances:

FirstChoice Uniform (now Regence)
I am out of network for other insurances. Going to an out of network provider usually results in a slightly higher out of pocket cost for the client.
I no longer accept Medicare, Medicaid, Value Options, Tri-Care, or United Healthcare.
Payment Policy:
I discuss my payment policy before the first visit, and make certain that we are in agreement, and that my services are affordable for you. Information is also provided in writing regarding my payment policies. Visa, Mastercard, Discover card, checks and cash are accepted.
